This is where Jim wakes up to doctor Livesey's arrival, and Silver greets the doctor, and Jim holds back a little bit, he was nervouse and afraid to face him.

I think the author was trying to set up a mood in which he wanted to make the readers think, and kind of get them scared with the personality of the doctor.

I don't believe Jim and the doctor would start anything because Stevenson didn't give Jim a big ego or didn't make him so arrogant; he knew when to and when not to react. This is probably so because he knew that Silver was there, and greeting the doctor would be sending Silver a message. Silver probably didn't want to make a scene because he knew that all others would attack. I feel that the author was trying to set a mood of suspense, to see what would happen next, and then to see where that would take the characters.
